Renewal Requirements:

• Annual fee of $200 (includes certification renewal for 2010 plus renewal of your web page on the FSS website forone year)

• Proof of insurance

• Any changes to your FSS website profile (the deadline for changes is Dec. 28)

• Submission of a one to two page report touching on the following:

            -Personal insights gained from facilitating Depth Hypnosis sessions

            -Courses of study you have undertaken to broaden your work and insights gained from                 them

            -Maintenance of and insights gained form your personal journeying and meditation                         practices
